Dr. Cotter has served as a Preceptor for the University of Alabama Birmingham Physician Assistant Program for 5 years. He currently employs two physician assistants in his private practice - one trained in dermatology and the other a surgical PA. For many years, Dr. Cotter has been an advocate for excellence in the training and use of physician assistants and nurse practitioners in healthcare. He advocates and teaches the use of Dr. Atul Gawande's "Checklist Manifesto" in conjunction with a definitive "time out" prior to surgical procedures and emphasizes early post-surgical communication with patients to prevent avoidable complications. Use of evidence-based guidelines for plastic & reconstructive surgical procedures is incorporated into daily teaching of students.

In June of 2014 we completed the required steps involved with a Region 3/Connolly Recovery Audit Contractor review of claims for medically necessary blepharoplasty. All charts requested were cleared with the exception of a single chart (that coincidently was the only chart that did not have an Advanced Beneficiary Notice). Within 72 hours after additional information was supplied, the negative finding for that case was reversed. Interestingly, in 2001, we had a similar pre-RAC experience regarding review of 106 charts for physician assistant "incident to" charges. Following three years of ever higher appeals including a final appeal to an administrative law judge we were able to reverse the initial negative findings and were cleared of virtually all negative findings. Dr. Cotter was Chief of the Department of Surgery for Shelby Baptist Medical Center, a two hundred bed hospital in rapidly growing Shelby County, Alabama from June 2007 to December 2012. During his tenure as Chief of Surgery operating room capacity was expanded from eight to twelve operating rooms including a major PACU expansion. Dr. Cotter supported the initiation of the cardiothoracic surgery program as well as robotic surgery and vascular surgery at Shelby Baptist. After a late 1999 Joint Commission survey, Shelby Baptist Medical Center was tasked with improving the credentialing of Allied Health Professionals working in the facility. Dr. Cotter organized and Chaired the 1st Allied Health Professionals Committee and wrote numerous Policies, Rules and Regulations for the facility. Many of these are now used by the entire Baptist Health System and others in the local healthcare community as standards for Allied Health Professional credentialing.

Timothy Cotter's presentation on Physician and Executive Compensation Models was eye-opening especially regarding the increasing linkage of compensation to the same value-based purchasing parameters we are all having to meet. Importantly, they deal with the challenges to building Trust and ways to go about building Trust that matters. Their book is particularly important, I believe, to the well-being and ability of medical staffs to work together among themselves and with their administrative partners as we move forward in this new healthcare milieu. I recommend it wholeheartedly, especially, to my colleagues interested in physician leadership, patient safety and healthcare quality improvement.
